Planning practical support after a hospital stay

Returning home may involve practical changes as well as a professional discharge plan. Keep those needs distinct when exploring support.

Agree discharge requirements first

The responsible professionals should establish the person’s discharge plan and healthcare requirements. Families may also wish to discuss companionship, shopping, meal preparation or other practical activities at home.

Check timing and responsibilities

Share the proposed postcode, activities and planned timing. Any personal-care, medication, mobility or specialist requirements need separate review. Do not rely on CarePal matching for urgent discharge cover, clinical recovery or an immediate start.

Choosing someone to support you

A CarePal is a care and support worker matched around the individual’s support needs, interests, routines, communication preferences, location and availability. You review suitable profiles, help choose whom to meet and meet a potential CarePal before support starts.

We aim for a dedicated CarePal, or a small consistent team where appropriate, so there is time to build familiarity. The same individual cannot be guaranteed for every visit. Discuss absence and cover arrangements before support begins.

Discuss whether we can help

Our minimum visit is two hours, with a minimum support package of six hours per week. There is no upfront matching fee. Payment begins when a suitable CarePal is in place and support starts, in line with your agreement.

Support is subject to suitability, agreed arrangements and CarePal availability. Any personal-care, medication or specialist requirements need a separate scope and suitability check before anything is agreed. This is planned regular support, not an emergency or guaranteed immediate-start service.
