Everyday support alongside a long-term condition

Care

A long-term condition does not tell us everything about a person’s interests, preferences or the support they want. Discuss everyday activities with the individual and keep clinical advice and treatment with the relevant professionals.

Practical support to discuss

Agreed help may include companionship, meal preparation, shopping, household tasks and practical accompaniment to appointments, subject to suitability and travel arrangements. Matching considers the person’s routine, preferred pace, location and availability.

We do not present CarePal matching as managing a medical condition, reducing hospital visits or preventing complications.

We take time to understand your support needs, interests, routines and communication preferences before sharing suitable CarePal profiles. You help choose whom to meet and meet before support starts. We aim for a dedicated CarePal, or a small consistent team where appropriate, so there is time to build familiarity and trust. The same individual cannot be guaranteed for every visit.

Tell us about any personal-care, medication or specialist requirements. These need a separate suitability and scope check under the current arrangements before anything is agreed.

Our minimum visit is two hours, with a minimum support package of six hours per week. There is no upfront matching fee. Payment begins when a suitable CarePal is in place and support starts, in line with your agreement.

Support is subject to suitability, agreed arrangements and CarePal availability. This is planned regular support, not an emergency or immediate-start service.
